20α-Hydroxycholesterol (20α-OHC) is a hydroxylated derivative of cholesterol, characterized by the presence of a hydroxyl (–OH) group at the 20th carbon position of the sterol backbone. It belongs to the oxysterol class, which are oxidized derivatives of cholesterol, and is produced through enzymatic processes involving cholesterol hydroxylases, such as cytochrome P450 enzymes, specifically CYP11A1, also known as cholesterol side-chain cleavage enzyme. 20α-OHC is primarily synthesized in steroidogenic tissues (such as the adrenal glands and gonads) and serves as an intermediate in the biosynthesis of steroid hormones.
Accurate quantification of 20α-hydroxycholesterol is crucial for diagnosing adrenal diseases that affect steroid hormone production. For example, abnormalities in the production or metabolism of 20α-OHC may indicate defects in the activity of steroidogenic enzymes (such as CYP11A1), leading to conditions like congenital adrenal hyperplasia.
